Rehabilitation Services
Injury or illness can have devastating effects on elders. At Maple Heights, we offer skilled rehabilitation services to help you or your loved one reach the maximum level of independence. Many residents rehab at Maple Heights and return home to live active lives. We offer all modalities of therapy including Physical, Speech, and Occupational Therapy. Maple Heights accepts Medicare Part A and Medicare Part B for rehabilitation services, as well as private pay, Medicaid and approved private insurances.

Assisted Living
Help from Friends
When you or a loved needs extra help with everyday activities, it may be time to consider Assisted Living. We offer individualized care designed to maximize independence and promote socialization with neighbors and the greater community. Assisted Living residents have full access to 24 hour nursing services, daily Registered Nurses and a wide variety of activities including bus rides, games, cooking, exercises and more.

Windmill Place
Dementia Care
What is Windmill Place? Windmill Place is a safe, secure living area for our elders living with dementia and those diseases resulting in dementia such as Alzheimer's and Parkinson's disease. Personalized activities, social interaction, higher staff to resident ratios, and engagement with daily life routines provides comfort for the residents. We strive to ensure that those with dementia can feel a sense of purpose.

Respite Care
Just for a Little While...
Family caregivers often neglect self-care while focusing on their loved ones needs. Planning a day trip or vacation can provide a chance for relaxation and rejuvenation. Maple Heights offers Adult Day Care and Respite Care to provide short-term care for elders. We offer medical services, special diets, snacks, grooming and activities.

Hospice Care
End of Life Care
Hospice Care is available for those who have been diagnosed with a terminal illness with less than 6 months to live. While Hospice is meant for end of life care, it isn't always a permanent choice. Some residents may get better during Hospice Care, while others decide on a specific time period for this service. The residents, their families and their physicians work together with the Maple Heights team and Hospice staff to provide the best care available for the elder. Maple Heights has Hospice contracts with all local hospice companies and encourage families to explore their options.
